We shall notify you should there be changes on the schedule of our dentist. Please note that this booking is good for 1 patient only. Should you want to book for another person, please fill up a new form.

 

For Patients with HMO Accredited card or plan, Please use the calendar intended for HMO patients.

 

For Virtual Consultation, Please select the "Consultation - Virtual/Online" and type your Contact details in the "Others" field (e.g. Messenger - @vreapor / Viber - 0917 111 1111).
