Thanks for booking an appointment with us! Please also note that we are strictly implementing the following rules in our clinic. Kindly read each item carefully. For any inquiries, please feel free to message us through our Facebook page.
Our slots are limited and are strictly scheduled to accommodate all patients.
Please bring your card if you're vaccinated. If not, please bring a certificate showing a Negative RT-PCR or Antigen Test Result. The result should be a maximum of 7 days prior to your appointment date.
PATIENTS WITH COVID-19 SYMPTOMS SHOULD RE-SCHEDULE
Should the patient acquire colds, cough, fever or had a recent contact with a person suspected of COVID-19, he/she should notify the clinic to re-schedule his/her visit
Our clinic has a parking provision for its patients. The parking spaces however are on a first come, first served basis.
Bringing of companions is highly discouraged to observe the social distancing inside the clinic. If the patient is a minor, 1 adult companion may come inside.
HMO PATIENTS SHOULD BRING THEIR HMO CARD AND VALID ID
For HMO Patients, please bring your HMO Card or Letter of Authority (for CARITAS Members) and a valid ID. Please note that benefits are non-transferable to another patient.
